WASHINGTON ? President Barack Obama is in excellent health and tobacco free, his doctor said Monday in the results of the president's second physical exam since taking office.

In the two-page report released by the White House, Dr. Jeffrey C. Kuhlman also said Obama is physically active, eats a healthy diet, stays at a healthy weight, and on occasion drinks alcohol in moderation.

"The president is in excellent health and `fit for duty,'" Kuhlman wrote. "All clinical data indicate he will remain so for the duration of his presidency."

Obama, who turned 50 in August, seems to have improved his health on some fronts since his last physical, which Kuhlman conducted in February 2010.

At that time, Kuhlman said Obama should stick with "smoking cessation efforts," and also said his cholesterol level had crept to borderline high and he should alter his diet accordingly.

Since then, Obama, who chewed nicotine gun, appears to have quit smoking entirely ? an achievement his wife, Michelle Obama, announced earlier this year. And his cholesterol is now described as "ideal."

Obama's total cholesterol level now stands at 193, compared with 209 in February 2010. His LDL, or bad cholesterol, is now 110, down from 138 last year.

Obama frequently works out and plays basketball and golfs.

The physical released Monday was conducted last week at the White House. Kuhlman, a Navy captain, recommended that the president have his next physical in December 2012.

The president was checked for and found free of colon cancer with a virtual colonoscopy last year, so he wasn't due to be tested again.

The new report says he was also screened for prostate cancer using a PSA blood test. That's a test that the U.S. Preventive Services Task Force recommended against earlier this month, saying it can do more harm than good in part because many tumors found are too slow-growing to be a threat. The report cites "informed patient request" in giving Obama the screening. His PSA level, or prostate-specific antigen, was found to be low.

The president is 6-foot-1 and weighs 181.3 pounds, according to the report. His blood pressure was recorded at 107 over 71, up from 105 over 62 in February 2010, but still well within the normal range.

The doctor said Obama's vision was 20/20 in both eyes for both distance and near vision.

The report found a "well-healed lower lip laceration" ? apparently a reference to an injury Obama sustained about a year ago when he was elbowed in the lip while playing basketball and had to get stitches.

The president also has "periodic physical therapy" to deal with recurrent upper right back pain, and he had benign skin tags removed from his neck.

Apple stuns Wall Street with rare miss, shares dive (Reuters)

SAN FRANCISCO (Reuters) ? Apple Inc reported a rare miss in quarterly results after sales of its flagship iPhone fell well short of Wall Street expectations, hammering its shares.

Shares of the world's most valuable technology corporation dived more than 5 percent to below $400 after it said it sold 17.07 million iPhones -- well short of the roughly 20 million expected by analysts.

Apple's CFO said iPhone sales came in ahead of internal expectations but were hurt in September by customers waiting for a new version.

The September quarterly report was Apple's first under new Chief Executive Tim Cook, who took over in August after co-founder Steve Jobs resigned. The company lost its leading visionary and co-founder when he died October 5.

Cook takes over during a critical juncture for the company, which is battling a fast-rising Google Inc in the mobile arena while fending off consumer electronics giants such as Samsung and Amazon.com Inc.

"Expectations for this company were red-hot, that is why we downgraded it," said BGC Partners analyst Colin Gillis, who lowered his rating on the shares days before. "The reality is their business is not an annuity. They have to sell their quarter's worth of revenue every 90 days."

"They had a big upgrade cycle with the iPhone, the numbers came in weak. They need to set records every time they report to keep up the momentum."

Apple said its revenue rose to $28.27 billion but that was also lower than the average analyst estimate of $29.69 billion, according to Thomson Reuters I/B/E/S.

The company reported a net profit of $6.62 billion, or $7.05 a share. That fell shy of expectations for earnings of $7.39 per share.

"There's no question this was a transition quarter ahead of the 4S," said WP Stewart portfolio manager Michael Walker. "With the early pace of iPhone 4S sales, my guess is that disappointment is relatively short-lived."

"I'm not going to call Q3 a throwaway quarter for iPhones, but it was definitely transition.

The company moved 4 million iPhone 4S units -- more than double its predecessor -- in its first three days, despite lukewarm reviews.

A PERIOD OF TRANSITION

A large spike in sales of Mac computers during the September quarter failed to lift earnings. Apple sold 4.89 million Macs, up 27 percent from a year ago. And it moved 11.12 million iPads.

Gross margin came to 40.3 percent -- a tad higher than Wall Street's forecast of 39.74 percent. International sales accounted for 63 percent of the quarter's revenue.

Some analysts had feared the lack of a new iPhone in more than 15 months would hurt the September quarter, while arguing the holiday season would turn out strong after the introduction of the fifth version of its smartphone.

"We expected iPhone sales to decline in the September quarter from the June quarter as a result of the announcements we made at WWDC in June, where we said we would launch iOS 5 and iCloud in fall," said Peter Oppenheimer, Chief Financial Officer, in an interview with Reuters.

"That basically created the rumor of the day across the September quarter, especially at the end."

Apple said it expected December quarter earnings of $9.30 a share on revenue of about $37 billion.

The forecast for the December quarter far surpassed Wall Street's projection for just above $9.00 -- a rare instance of its outlook exceeding expectations. Apple typically guides so conservatively that investors disregard its forecasts.

But that failed to discourage the after-hours sell-off. Before the numbers, Apple's shares had climbed to as high as $425 in extended trade.

"What is interesting is the guidance is less conservative than usual for their next quarter. It's a timing issue, where it looks like the business that people thought would be in the September quarter is occurring in the December quarter," said Sterne Agee analyst Shaw Wu.

"One of the things obviously is the iPhone 4S just started shipping a few days ago."

TORONTO (Reuters) ? An offer of free games, translation software or other apps to compensate BlackBerry users for last week's prolonged outage left Research In Motion investors cool on Monday, and the shares fell 6 percent.

RIM declined to say if it would need to amend its earnings forecasts to account for the cost of its promise to give $100 of free apps to every BlackBerry smartphone user.

RIM is also offering a period of free technical support to businesses that use the gadget, which has steadily lost market share to Apple's sleeker, sexier iPhone. RIM's stock has dropped 60 percent over the past year.

"RIM has responded swiftly but this won't undo the damage done to its reputation," analyst Geoff Blaber at CCS Insight told Reuters earlier on Monday. "This may go some way to appeasing customers but what's critical is that the problem does not repeat itself."

Highlighting the challenges, Apple said it sold 4 million of its new iPhone 4S in the three days after its launch last week.

Tens of millions of BlackBerry users were left without mobile email and other messaging for up to four days last week after a failure at a RIM data center in England triggered a service disruption across five continents.

DEVELOPERS CONFERENCE

RIM may reveal more about its strategy for countering the competitive challenge at a conference for application developers in San Francisco that begins Tuesday.

RIM executives there are expected to unveil a major software upgrade for the PlayBook tablet computer. RIM may also provide a glimpse at next-generation smartphones using its QNX software, which already powers the PlayBook.

Long before last week's disruption, investor dissatisfaction with the management of co-chief executives Mike Lazaridis and Jim Balsillie had led a wave of speculation about the future of the company.

Activist investor Carl Icahn squelched one of those rumors on Monday when he told broadcaster CNBC that RIM is not on his radar screen. Last month RIM's shares rose on speculation Icahn would buy into RIM and agitate for change.

Even so, a smaller activist firm, Jaguar Financial Corp, has said it is gathering the support of large RIM shareholders to push the board to look at strategic options, including a sale or split-up of RIM.

"DEEPLY GRATEFUL"

Last week's outage intensified criticism of the chief executives, who were accused of responding slowly to the crisis and communicating poorly.

On Monday Balsillie told Reuters the company wanted to make amends with customers.

"This is our way of expressing appreciation for their patience during the recent service disruptions and a tangible way of telling them how deeply grateful we are for their continued business," he said in a phone interview.

Balsillie declined to estimate how much the offer would cost RIM and said he was unable to say whether RIM might have to revise its earnings forecast for the current quarter, which ends in late November.

The financial impact could prove sizable if a sizable number of RIM's more than 70 million subscribers take up the offers.

Analysts have said compensation costs could reach into the hundreds of millions of dollars. If RIM were to pay back all carriers and customers for lost service it could knock between 3 and 5 cents off earnings per share in the quarter, according to BMO Capital Markets analyst Tim Long. That would reduce profit by $15 million to $26 million.

"CLEVER MOVE"

Even so, Richard Levick, who runs a U.S. consultancy that specializes in crisis management, praised the free-app offer but said RIM should have made the announcement last week.

"I think it's a good start, but they are always late," he said. "They are always behind the curve."

Francisco Jeronimo, an analyst at IDC, said the offer was a clever move by RIM because it would help customers to discover the app service. He said the company was likely to have struck a deal with app developers to keep the cost down.

"For RIM, this is an interesting way to attract users to the App World and incentivize them to search and download apps," he said.

The free apps on offer include games such as Bejeweled, and premium versions of a translation service and the music discovery tool Shazam. Users can download them from BlackBerry App World beginning Wednesday, with more to be added in the next four weeks. The offer runs until the end of the year.

"More important than the offer itself, is that RIM is showing goodwill and being humble," Jeronimo said. "They recognized the problem, apologized and now they are compensating their users."

By early afternoon the stock dropped about 6 percent at $22.54 on the Nasdaq.

U.S. deports record number of immigrants in 2011 (Reuters)

WASHINGTON (Reuters) ? The United States deported nearly 400,000 illegal immigrants in fiscal 2011 -- the highest number since the Immigration and Customs Enforcement agency was formed eight years ago.

About 396,906 foreign nationals were deported in the fiscal year ending September 30, the agency said on Tuesday.

This compares to about 393,000 removed in fiscal 2010.

The figure highlights tensions in President Barack Obama's attempts to ease deportations, which have been steadily increasing, and his desire to formulate a more popular immigration policy ahead of 2012 elections.

Illegal immigration is a hot political issue in the United States. More than 11 million illegal immigrants live and work in the United States, according to the Pew Hispanic Center.

States including Arizona and Georgia have tried to enact their own laws cracking down on illegal immigration, arguing that the federal government has done too little to stop it.

While Obama has failed so far to get Congress to pass comprehensive immigration reform, he ordered the Department of Homeland Security in August to prioritize deportations of people who faced criminal charges -- a move viewed by analysts as a way to appease advocates of more liberal immigration laws.

In the 2008 election, 67 percent of Hispanics voted for Obama over his Republican rival John McCain.

"These year-end totals indicate that we are making progress, with more convicted criminals, recent border crossers, egregious immigration law violators and immigration fugitives being removed from the country than ever before," said John Morton, director of ICE.

Just over half of those removed from the country had been convicted of crimes related to drugs, driving under the influence, homicide or sex offenses.

Slightly more than 142,000 of the deportations were immigrants the agency describes as "recent border crossers" or those who had repeatedly broken immigration law.

The agency credited the increased deportations to the expansion of programs like the Secure Communities Program where local authorities send fingerprints of those arrested to ICE. The program has grown under Obama from 14 jurisdictions to more than 1,300.

New ADHD Guidelines Include Preschoolers, Older Teens (HealthDay)

SUNDAY, Oct. 16 (HealthDay News) -- In new guidelines released Sunday, the American Academy of Pediatrics has expanded the age range for the diagnosis and treatment of attention-deficit hyperactivity disorder (ADHD) to children as young as 4 and as old as 18.

For the youngest children, the academy is emphasizing the use of behavior treatments over medication in most cases.

"I think the most significant changes are expanding the ages from preschool through adolescence. The original guidelines were from 6 to 12, because that's where the evidence was. We've been able to broaden the scope of the guidelines because there was more evidence available for preschoolers and adolescents," said the lead author of the new recommendations, Dr. Mark Wolraich, CMRI Shaun Walters Professor of Pediatrics and the Edith Kinney Gaylord Presidential Professor at the University of Oklahoma Health Sciences Center in Oklahoma City.

Wolraich added that the new guidelines also give pediatricians advice on managing inattention or hyperactivity problems that don't quite meet the definition of ADHD.

The new guidelines will be presented at the academy's annual meeting in Boston and will be published in the November issue of Pediatrics.

More than 5 million children in the United States have been diagnosed with ADHD, according to the U.S. Centers for Disease Control and Prevention. Children with the disorder show signs of inattentiveness, impulsivity and hyperactivity. They may be unable to pay attention in class, or may spend a lot of time fidgeting in their seats or talking nonstop. Although most kids may display this type of behavior at one time or another, it becomes a problem when it occurs most of the time, according to the U.S. National Institute of Mental Health.

Treatment for ADHD may include medications or behavior therapy, or both. In fact, Wolraich said that "the combination of both medication and behavioral therapy is probably the best choice when possible."

A recent study, published online on Sept. 28 in the American Journal of Psychiatry, found that the use of ADHD medication is on the rise, with 5 percent of American children now taking stimulant medications such as Ritalin or Adderall to treat the disorder. Those researchers suggested that the increase might be due to a greater recognition of ADHD as a chronic condition, leading to children staying on medications for longer periods.

The new guidelines recommend that unless a child in the 4- to 6-year-old age group has a serious problem, that behavior therapy should be the first treatment tried. If necessary, medications can be added later.

"Although there is less evidence [on outcomes for this age group], the hope is that starting treatment in preschool if a child has serious problems will lead to better outcomes," said Wolraich.

Dr. Richard Gallagher, director of special projects at the Institute for Attention-Deficit Hyperactivity and Behavior Disorders at the NYU Child Study Center in New York City, said, "Medications should be used very carefully. I'm admittedly biased since I primarily do behavioral work, but behavioral work does have its limits. If a child is getting into dangerous situations or presenting with problems interacting appropriately with peers and adults, medications can be very useful. When monitored carefully, medications are safe for the vast majority of kids."

The guidelines also emphasize the need for pediatricians to recognize that ADHD is a chronic condition, and while treatments are available to control symptoms, there is no cure for the disorder.

Wolraich said that one of the reasons the AAP expanded the age up to 18 is because there have been more studies showing that ADHD continues into late adolescence, and even into adulthood.

"ADHD is a chronic condition. We can provide symptomatic treatment, but it doesn't cure the condition. Treatment has to be an ongoing process. Symptoms may change over time, and there needs to be consideration for change in treatment as a child develops," said Wolraich.

"Kids usually stay on medications for about three years. For some, that's enough. It gives them time to become better at compensating for their deficits. But, for many kids, the need for treatment is ongoing," he added.

Gallagher recommended that parents bring up any concerns they have about their child with their pediatrician. "This is a condition that can be recognized early," he said.

WASHINGTON (AP) ? Regulators on Friday closed small banks in Georgia, North Carolina, New Jersey and Illinois, boosting to 80 the number of U.S. bank failures this year.

The number of closures has fallen sharply this year as banks have worked their way through the bad debt accumulated in the recession. By this time last year, regulators had shuttered 132 banks.

The Federal Deposit Insurance Corp. seized Piedmont Community Bank of Gray, Ga., with $201.7 million in assets and $181.4 million in deposits. It also shuttered Blue Ridge Savings Bank, based in Asheville, N.C., with $161 million in assets and $158.7 million in deposits.

Also closed was First State Bank in Cranford, N.J., with $204.4 million in assets and $201.2 million in deposits, and Country Bank in Aledo, Ill., with $190.6 million in assets and $167.5 million in deposits.

State Bank and Trust Co., based in Macon, Ga., agreed to assume the assets and deposits of Piedmont Community Bank. Bank of North Carolina, based in Thomasville, N.C., is acquiring the assets and deposits of Blue Ridge Savings Bank, and Northfield Bank, based in Staten Island, N.Y., is assuming those of First State Bank.

Blackhawk Bank & Trust, based in Milan, Ill., is assuming all the deposits and $113.3 million of Country Bank's assets. The FDIC will retain the rest for eventual sale.

In addition, the FDIC and State Bank and Trust agreed to share losses on $163.2 million of Piedmont Community Bank's loans and other assets. The agency and Bank of North Carolina are sharing losses on $143.2 million of Blue Ridge Savings Bank's assets.

The failure of Piedmont Community Bank is expected to cost the deposit insurance fund $71.6 million. That of Blue Ridge Savings Bank is expected to cost $38 million, First State Bank, $45.8 million, and Country Bank, $66.3 million.

Georgia and Illinois have been among the hardest-hit states for bank failures. Regulators closed 16 banks each in those two states last year. Piedmont Community Bank is the 20th Georgia lender shut down this year, while Country Bank is the eighth in Illinois.

California and Florida also have seen large numbers of bank failures.

In all of 2010, regulators seized 157 banks, the most in any year since the savings and loan crisis two decades ago. Those failures cost around $23 billion. The FDIC has said 2010 likely was the high-water mark for bank failures from the Great Recession.

In 2009, there were 140 bank failures that cost the insurance fund about $36 billion, a higher price tag than in 2010 because the banks involved were bigger on average. Twenty-five banks failed in 2008, the year the financial crisis struck with force; only three were closed in 2007.

From 2008 through 2010, bank failures cost the fund $76.8 billion. The FDIC said this week it expects failures from 2011 through 2015 to cost $19 billion.

The deposit insurance fund fell into the red in 2009. With failures slowing, the FDIC's fund balance turned positive in the second quarter of this year; it stood at $3.9 billion as of June 30.

Depositors' money ? insured up to $250,000 per account ? is not at risk, with the FDIC backed by the government. That insurance cap was made permanent in the financial overhaul law enacted in July 2010.

Beyond the obvious trends, there are a few ongoing details that span the collections in the shops right now and the ones that will appear in six months' time. Curved shoulders, often constructed with a raglan sleeve (in which the sleeve goes in one piece from the cuff to the neckline, rather than having a classic "socket"), are probably most iconically found on the Dior Bar jacket, as reprised by Bill Gaytten this season, but can be seen across the collections as an easy, sophisticated alternative to padded shoulders. Shoe heights have continued to shrink, and although there are still plenty of platforms out there (the most popular for summer being the block-platform sandal), flats and kitten-heels are making incursions, as are rather elegant stilettos. Trousers ranged from clown-wide (at Kenzo) to cigarette-thin (at Yves Saint Laurent), but while you might not always look to Italy for innovation, its collections will usually offer the most flattering version of any prevailing look, and Giorgio Armani's trousers were just that: slender but not tight, cropped a fraction above the ankle and loose in the hips.

It started a few seasons ago, at Dior and Louis Vuitton, but that full-skirted Mad Men look has developed into something altogether less "costumey" and more relaxed.

TUESDAY, Oct. 4 (HealthDay News) -- New research shows that the few men who develop breast cancer tend to have more advanced cases than women and to be diagnosed at an older age.

But when statistics are adjusted for factors such as age, men with breast cancer are less likely to die from the disease than women are.

The findings shed light on a rare disease in men, one that researchers had earlier assumed was deadlier for males.

"Men can develop it and should be aware that they should seek care if a breast lump develops," said study co-author Dr. Mikael Hartman, an assistant professor at National University of Singapore.

About 2,140 men in the United States will develop breast cancer this year, according to an estimate from the American Cancer Society (ACS), and about 450 men will die from it. The ACS estimates that the lifetime risk that a man will develop breast cancer is one in 1,000, although the likelihood skyrockets to 5 percent to 10 percent if a man has a mutation in a gene known as BRCA2, noted Dr. Mahmoud El-Tamer, an attending surgeon at Memorial Sloan-Kettering Cancer Center who studies male breast cancer.

The same mutation greatly boosts the risk of breast cancer in women.

It's not clear why breast cancer is much less common in men. Women, of course, have much more breast tissue. But volume doesn't appear to affect the risk of breast cancer in women, since those with large breasts don't develop the disease more than those with small breasts, El-Tamer said.

Estrogen could be key, study co-author Hartman said, since it seems to fuel breast cancer and is almost entirely absent in men.

In the new study, which appears online Oct. 3 in the Journal of Clinical Oncology, researchers analyzed statistics on breast cancer in both sexes in Denmark, Finland, Switzerland, Norway, Singapore and Sweden over the past 40 years. The researchers found 459,846 cases of breast cancer in women and 2,665 in men.

Women were diagnosed at age 62 on average and men at age 70. Men with breast cancer were less likely to live for five years than women with the disease were, but the situation reversed when researchers adjusted their statistics so they wouldn't be thrown off by differences between the two groups in terms of age and other factors.

Hartman said it's not clear why men do better, but it may have something to do with how their bodies react to anti-hormone and chemotherapy treatments. (Removal of the breast is another treatment for men with breast cancer.)

Over time, the prospects for women with breast cancer have improved at a greater rate than those for men, he said.

El-Tamer said men should be aware of the possible risk of breast cancer and see a doctor if they notice a lump or changes involving the nipple, such as an ulceration, discharge or changes in the areola.

TAMPA, Fla. (AP) ? Josh Freeman and LeGarrette Blount helped the Tampa Bay Buccaneers rally for another victory.

Freeman threw for 287 yards and a touchdown, Blount ran for 127 yards and a score, and the Buccaneers beat the Indianapolis Colts 24-17 on Monday night.

Freeman, who ran for a touchdown early in the game, scrambled for a huge first down on the winning drive as the Buccaneers (3-1) won their third consecutive game. Freeman rallied the Bucs to victory at Minnesota in Week 2.

The Colts fell to 0-4 for the first time since 1998, Peyton Manning's rookie season. Manning has watched all four games, unable to play because of a neck injury.

Curtis Painter started this one, filling in for injured backup Kerry Collins, who's out because of a concusssion, and played better than most expected. Painter threw for 281 yards and two touchdowns, both long ones to Pierre Garcon. He wasn't nearly as effective down the stretch, partly because the Colts lost starting offensive linemen Anthony Castonzo and Ben Ijalana to injuries. Guard Ryan Diem was inactive.

Painter completed 13 of 30 passes. Garcon turned a short sideline route into an 87-yard score in the second quarter, then took a screen pass and weaved his way through defenders for a 59-yarder in the third.

The 87-yarder was the team's longest play of the season, the fifth-longest pass play in team history and longer than anything Manning has thrown in his NFL career.

The Colts were up 17-10 after Garcon's second score, but it was all Tampa Bay in the final 20-plus minutes.

Freeman was 25 of 39 passing, buying time in the pocket and picking apart Indy's zone defense. He got the Bucs on the scoreboard with a 1-yard plunge in the second quarter and tied the game at 10 with a 13-yard pass to Preston Parker late in the third.

Blount put them ahead for good when he broke through the line of scrimmage, cut right and scampered for a 35-yard score with 3:15 remaining. Freeman had a huge run on a third-and-6 play on the drive. He eluded a rusher, scrambled out of the pocket and dived for a first down.

Freeman also converted a fourth-and-1 play with 1:10 remaining, allowing the Bucs to run out the clock. He finished with six carries for 27 yards.

Tampa Bay's biggest problem ? aside from the two long TD passes ? was penalties. The Buccaneers were flagged 14 times for 106 yards.

None of those was as costly as Dwight Freeney's penalty. Indianapolis' star pass-rusher was flagged for being offside on a third-and-19 play. The Buccaneers converted on the ensuing play, setting up Connor Barth's 46-yard field goal that tied the game at 10.

Indy went back in fronton Garcon's second score. He eluded several defenders while crisscrossing the field, and may have been aided by a block in the back. There were no flags, though.

Garcon's first one was nearly as good. He turned up the sideline and outran everyone to the end zone, giving the Colts a 10-0 lead.

Freeman responded by driving the Bucs down the field, then sneaked the ball across the goal line. Tampa Bay had a chance to tie the game or take the lead just before halftime, but Robert Mathis sacked Freeman on third down and the Bucs had to scramble to get the field goal unit lined up.

They did, and Barth's kick was good from 37 yards. But the Bucs had 12 men on the field, so they went to the locker room trailing.

It didn't matter, not against the struggling Colts.

With Collins out, Painter made his first career start, and recently signed Dan Orlovsky served as the backup.

Indy looked like it might pull the upset until the linemen starting going down. The Colts also lost defensive tackle Eric Foster with a dislocated right ankle.

Foster injured his ankle with 52 seconds remaining in the first half. He got tangled up in a pile while trying to make a tackle. Athletic trainers rushed to help him. Teammates and opponents prayed and Colts coach Jim Caldwell knelt by his side.

Foster was fitted with an air cast and taken to a hospital where he was to be kept overnight for observation. He pounded his chest and yelled to teammates as he left the field to applause.

Foster, out of Rutgers, is in his fourth season. He entered the game with 12 tackles.
